Insurers must publish their provider directories. Every month we read all of them and grade each plan, county by county.

Directory Integrity Scores for every federal marketplace plan publish here on October 26, 2026, before open enrollment begins. Every insurer receives its complete findings at least 14 days before publication.

What will be here

• A grade for every marketplace plan's mental health provider list, in every county it covers, computed from the insurer's own federally required data file.
• A ZIP code lookup, so you can see every plan's grades where you live.
• The evidence behind every grade, downloadable, with the source file's fingerprint on every row and a published manifest saying which address that fingerprint came from and when.
• A plain-language guide to demanding care when a plan's list fails you.
